To League Presidents,
Please circulate this draft copy of the Proxy form to the member clubs in your league/Association. This proxy is meant for the individual member clubs in your league that may not be able to send one of their officers to attend the Extra Ordinary General Meeting of USACA to be held on June 4, 2005. An officer of the member club has to fill out the proxy form and thereby authorize you or any other representative of their choice.

We the members of the ........................................ cricket club, a member of USACA participating in the ......................................... cricket league/association in the ...............................region, hereby authorize M/s..................................................................... to act as our proxy and to vote on any resolutions, including, but not limited to, resolutions for amendments of the USACA constitution, that may arise during the Extra Ordinary General meeting of the USACA members to be held on June, 4, 2005 in New York.
